Requirements for Travel Document application
- complete name of the representative
- the reason for inability to appear personally;
- personal signature of the applicant, or the parent of the applicant; and
- a copy of the representative’s passport data page or civil/resident’s ID (front and reverse sides) must be attached to the letter of authorization.
- Accomplished Affidavit of Lost Passport; and
- Police Report on my lost passport with English translation.
- Birth Certificate issued by the Philippine Statistics Authority (PSA) and apostilled by the Department of Foreign Affairs (DFA); and
- If the applicant is married and wishes to use the husband’s surname: Marriage Certificate issued by the PSA and apostilled by the DFA.
- Accomplished Affidavit of Lost Passport; and
- Request for Assistance Form (RAF).

Do not lose the original Travel Document issued by the Philippine Embassy in Oman. It is among the requirements for applying for a new passport.

In obtaining a Travel Document, the applicant will not be able to renew his/her passport in the Philippines. The applicant must submit the requirements for a new passport, or for the replacement of a lost passport, together with the original Travel Document.
