Requirements for Travel Document application

1. Personal appearance, for adult applicant or on behalf of his or her minor child

If an adult applicant cannot personally appear at the Embassy, or on behalf of his/her minor child, the applicant must submit a letter of authorization of the applicant’s representative stating the following:

  • complete name of the representative
  • the reason for inability to appear personally;
  • Signature of the applicant, or the parent of the applicant; and
  • a copy of the representative’s passport data page or civil/resident’s ID (front and reverse sides) must be attached to the letter of authorization.

2. Accomplished passport application form (New – Adult or Minor Applicant) to be signed by the applicant; or by the applicant on behalf of his/her minor child

3. Original latest passport and photocopy of its data page, if available

a. For lost original passport, but a copy of its data page is available, additional requirements:

  • Accomplished Affidavit of Lost Passport; and
  • Police Report on lost passport with English translation.

b. For lost original passport and no available copy of its data page, additional requirements:

  • Birth Certificate issued by the Philippine Statistics Authority (PSA) and apostilled by the Department of Foreign Affairs (DFA); and
  • If the applicant is married and wishes to use the husband’s surname: Marriage Certificate issued by the PSA and apostilled by the DFA.

c. For missing latest original passport, but applicant has been approved for the amnesty program of the host government and not returning to Oman, additional requirements:

  • Accomplished Affidavit of Lost Passport; and
  • Request for Assistance Form (RAF) to be accomplished in the Embassy
  • Endorsement from the Migrant Workers’ Office for the issuance of a travel document 

4. Copy of valid airline ticket (for flight scheduled less than one month from the date of application for a travel document)

5. Four (4) pieces recent photos (2x2” size) with white background

6. OMR 12.000 (cash only).

Applicants for Travel Documents are reminded:

  • Do not lose the original Travel Document issued by the Philippine Embassy in Oman. It is among the requirements for applying for a new passport.
  • In obtaining a Travel Document, the applicant will not be able to renew his/her passport in the Philippines. The applicant must submit the requirements for a new passport, or for the replacement of a lost passport, together with the original Travel Document.
