Requirements for Travel Document application

 
1. Personal appearance, for adult applicant, or on behalf of his or her minor child
 
    If an adult applicant cannot personally appear at the Embassy, or on behalf of his/her minor child, the applicant must submit a letter of authorization the applicant’s                        representative stating the following:
 
  •  complete name of the representative
  •  the reason for inability to appear personally;
  •  personal signature of the applicant, or the parent of the applicant; and
  •  a copy of the representative’s passport data page or civil/resident’s ID (front and reverse sides) must be attached to the letter of authorization.

3.Original latest passport and photocopy of its data page
 
   a. For lost original passport, but a copy of its data page is available, additional requirements:
 
  • Accomplished Affidavit of Lost Passport; and
  • Police Report on my lost passport with English translation.
    b. For lost original passport and no available copy of its data page, additional requirements:
 
  • Birth Certificate issued by the Philippine Statistics Authority (PSA) and apostilled by the Department of Foreign Affairs (DFA); and
  • If the applicant is married and wishes to use the husband’s surname: Marriage Certificate issued by the PSA and apostilled by the DFA.
    c. For missing latest original passport, but applicant has been approved for the amnesty program of the host government and not returning to Oman, additional requirements:
 
  • Accomplished Affidavit of Lost Passport; and
  • Request for Assistance Form (RAF).
 4. Copy of valid airline ticket (for flight scheduled less than one month from the date of application for a travel document)
 
 5. Four (4) pieces recent photos (2x2” size) with white background
 

 6. OMR 12.000 (cash only).

ADVICE
  • Do not lose the original Travel Document issued by the Philippine Embassy in Oman. It is among the requirements for applying for a new passport.
  • In obtaining a Travel Document, the applicant will not be able to renew his/her passport in the Philippines. The applicant must submit the requirements for a new passport, or for the replacement of a lost passport, together with the original Travel Document.

Applying for NBI Clearance

REQUIREMENTS (for first-time applicants living/working abroad):

  • Ask for a Fingerprint Card form from the Embassy and fill it up. For married women, please write your name in the following order: maiden surname (father’s surname), husband’s surname, first or given name, and maternal surname (mother’s family name).
  • Attach a colored 2” x 2” photograph, taken within three months before the date of application, showing a clear front view of applicant’s face, with a white background. No sleeveless attire. Blurred or low quality photos are not accepted.
  • The Embassy will have your fingerprints impressed (Rolled Impression) on the appropriate spaces of the NBI Fingerprint Card form.
  • The Embassy will authenticate the Fingerprint Card form. There is a fee for this service (please refer to fee/processing days below).
  • Send the authenticated Fingerprint Card form to the NBI for issuance of clearance (please refer to sending instructions below).

Fee / Processing Daysdocument authentication in the Embassy:

  • 10 Omani Rials (3 working days)
  • 14 Omani Rials (expedite - if to be filed before 12 noon, the release will be in the afternoon of the same day; but if to be filed between 1:00 - 3:00 pm, the release will be in the morning of the following day.

Sending Instructions:
By Mail - Enclose Money Order in the amount of Php 200.00 or bank draft, which is negotiable in the Philippines, payable to the Director, National Bureau of Investigation, Taft Avenue, Ermita 1000 Manila, Philippines. The following are the accredited banks:

Through a Representative in the Philippines – Mail the accomplished form to your representative, along with an authenticated signed letter or Special Power of Attorney (SPA) authorizing him/her to transact business on your behalf. Your representative may visit the office of the Electronic Data Processing Division (EDPD) in NBI Taft Avenue, Manila for further assistance.

Upon receipt of your clearance certificate (whether directly from the NBI or your representative), please examine it for the presence of the embossed NBI DRY SEAL at its lower left portion. The clearance is invalid without it. You must also affix your clear and properly taken THUMBPRINT on the space provided for on the clearance certificate before submitting it to the end-user, otherwise, your clearance will be subjected for further verification/authentication.

REQUIREMENTS (for applicants living/working abroad renewing their NBI clearance, issued from 1998 onwards):

  • Original NBI clearance, with updated information for address and purpose of clearance indicated; otherwise, write “Same Data”, if none.
  • For changes in name, date of birth or place of birth, duly-accomplished Fingerprint Card form, with photocopy of Birth Certificate or Marriage Certificate.
  • One (1) colored photo, 2” x 2”, taken within three months before the date of application, showing a clear front view of applicant’s face, with a white background. No sleeveless attire. Blurred or low quality photos are not accepted.
  • Photocopy of your passport with date stamped proving your presence abroad.
  • Send all of the above documents to the NBI, following the same SENDING instructions.

Note: If NBI clearance was issued prior to 1998, or you have lost your personal copy of the certificate, follow the instructions for First Time applicants from abroad.

Expedite Processing may be requested in Urgent Cases.  However, an Overtime Fee of O.R. 4 per document shall apply.

Citizenship Retention & Re-acquisition (Dual Citizenship)

Former Filipinos may re-acquire their Philippine citizenship through Republic Act (RA) No. 9225 or the Citizenship Retention and Re-acquisition Act. This law, which took effect on 17 September 2003, declares that former natural-born Filipino citizens who acquired foreign citizenship through naturalization are deemed not to have lost their Philippine citizenship under conditions provided in the Act. A former Filipino can re-acquire Philippine citizenship by taking the oath of allegiance to the Republic of the Philippines.

In view of the foregoing, the applicant is required to submit the following:

  • Duly accomplished Petition under Oath
  • Birth certificate issued by the NSO duly authenticated by DFA. In case the birth certificate is not available, other documentary proof of the applicant's being a former natural-born Filipino citizen.
  • Old Philippine Passport
  • Foreign Passport
  • Certificate of Naturalization. Since the Act requires that the "loss of Philippine citizenship by the applicant must be on account of naturalization as a citizen of a foreign country", the applicant should submit documents showing the fact of his naturalization as a citizen of a foreign country.
  • Authenticated marriage certificate, if married
  • Authenticated death certificate, if widowed
  • Annulment, if the marriage is annulled. The court decision to nullify the marriage must be certified by the proper court and authenticated by DFA.
  • Divorce decree, if divorcee. It must be certified by the proper court.
  • Three (3) 2 X 2 photos (white background) with the following view: left side view, front view and right side view
  • Payment of consular service fee of RO. 20.000 for taking an Oath of Allegiance for the re-acquisition of Philippine Citizenship

Note:

  • If an applicant for dual citizenship would like to include their unmarried minor (below 18 years of age) children who were born when both parents were already non-Filipinos: submit a duplicate original or certified photocopy of the birth certificate of your children and their foreign passport.
  • A child who is already 18 years old and above and was born when either of the parent was still a Filipino needs to apply for retention of citizenship on his/her own behalf.
  • Order of Approval will be issued by the consular officer of the embassy which shall be the substantive proof that applicant has re-acquired/retained Philippine citizenship.
  • Oath of Allegiance to be undertaken by the consular officer after the Order of Approval is issued.
  • The processing of the application will take after (5) working days.
