Report of Birth (ROB)

  • Duly accomplished Report of Birth form, (must be made in 4 copies, signed by either one of the Filipino parent).
  • Photocopy of parents' passport and labor card (4 copies).
  • Authenticated Marriage Contract of parents-National Statistics Office (NSO), Department of Foreign Affairs (DFA) - (4 copies).
  • Birth Certificate of child in English translation duly attested by the Oman Ministry of Foreign Affairs (Original and 3 photocopies).
  • Fee of R.O. 10.000.
  • In case of late registration of birth (more than 30 days from the date of issuance of birth certificate), additional fee of R.O. 10.000 will be charged.

Report of Marriage (ROM)

Marriages of Filipino citizens solemnized outside the Philippine Embassy or Consulate General must be reported to the Foreign Service post with jurisdiction over the town/city/country where the marriage was performed. The Report of Marriage will then be sent to the Department of Foreign Affairs in Manila for onward transmittal to the Office of the Civil Registrar General.

Requirements:

  • Duly Accomplished Report of Marriage Form
  • Marriage Certificate with English translation duly attested by the Oman Ministry of Foreign Affairs (Original and 3 photocopies)
  • Original Copy of the Legal Capacity to Contract Marriage issued by the Embassy if secured prior the marriage or the Certificate of No Marriage Record issued by NSO duly authenticated by DFA.
  • Copy of passport and labor card of contracting parties (3 copies).
  • Copy of the passport and labor card of the witnesses (3 copies).
  • Fee R.O. 10.000

PASSPORT FREQUENTLY ASKED QUESTIONS (FAQs)

Q: When should I renew my passport?

A: A passport with at least six (6) months’ validity is a standard requirement for international travel. In view of the long turnaround for the processing of a Philippine passport (around two months), Filipinos living overseas are therefore advised to renew their passports several months before expiry. We recommend eight months, or preferably, 9 months before its expiration to account for any possible delays in the transmission of the passport to the Embassy.

While the passport is being processed, you will continue to hold on to your present passport. This will ensure that you will be able to leave Oman for any emergency. Your present passport will be cancelled only when the new passport has arrived. When claiming the new passport, please bring the old passport as it will be cancelled in front of you. Filipino professionals who go on frequent travels are advised to submit their applications even earlier, and have their passport renewed long before all the pages have been used up.

There is no option for an applicant to apply for a passport with more than the standard number of pages under the current Philippine passport system; the number of pages is fixed.

Please be informed also that due to technical problems at the processing end in Manila, delays in the transmittal of printed passports to the Embassy occur occasionally. The Embassy is not informed by Manila as to the exact date that a (particular) passport will be sent to the Embassy, due to the way the system operates. Rest assured, however, that efforts are underway in Manila to resolve this situation of delay in the processing of certain passports.

Q: I am in urgent need of a passport and cannot wait for eight weeks. Please “expedite” my passport.

A: The Embassy regrets to inform you that there is no “expedite” process for Philippine Foreign Service Posts. Since all Philippine passports are printed in Manila, the Embassy is unable to accommodate “expedite” requests. In other words, the processing time cannot be shortened on a personal request. However, while the new passport is being made in Manila, you will continue to hold your present passport, which means you can still use it for employment purposes or for travel, if it is still valid.

Q: Can I request for the extension of my passport’s validity?

A: Passport holders are reminded that there are strict rules for applying for an extension of the validity of expiring passports. Passports that are valid for less than six (6) months may be extended once on the following instances:

  • death in the family requiring the OFW and members of his/her dependent family to urgently travel to the Philippines;
  • medical emergencies requiring the OFW and members of his/her dependent family to urgently travel to the Philippines or another country for medical treatment;
  • OFWs returning to their employers abroad with valid employment contracts processed by the Philippine Overseas Employment Administration (POEA) or any Philippine Overseas Labor Offices (POLO); and
  • those going home on final exit visas.

In these instances, proof of urgency such as a copy of the death certificate, medical certificate, valid employment contracts processed by the POEA or any of the POLO, along with plane tickets with confirmed flight details should be presented.

Per the regulations of the Department of Foreign Affairs (DFA) and the standards set by the International Civil Aviation Organization (ICAO), all non-machine readable passports may no longer be extended beyond October 31, 2015 and must be completely phased out by November 24, 2015.

All Filipino nationals holding Machine Readable-Ready Passports (MRRP; green passports) and Machine Readable Passports (MRP; maroon passports) will no longer be allowed to apply for an extension of the validity of these passports after October 31, 2015.

They must instead apply for a new e-Passport as soon as possible before the expiry of their current MRRP (green) or MRP (maroon) passports. Those who fail to do so will likely encounter difficulty at immigration checks when traveling through any ports of entry around the world after October 2015.

Q: How would I know if my passport has arrived already from Manila?

A: You will receive an SMS notification from the Embassy once your passport is ready for release. Please refrain from following up if only six (6) weeks has passed. As mentioned at the very beginning of the application process, the normal processing time for a passport to arrive from Manila is eight (8) weeks. After seven (7) weeks, however, you may wish to follow up with the Consular Section. Q: I cannot personally collect my passport. Can someone collect it for me?

A: Yes, you may authorize someone else to collect it for you, as long as you issue an authorization letter, with a photocopy of your ID or old passport, plus your present passport (which will be cancelled).

Requirements for Renewal of Passport

  • The biometrics of the passport applicant must be captured live. This is required even for infants.

3. Duly accomplished application form

  • Download, print-out and bring the correct application form:
  • Passport renewal for adult applicants 19 
  • Passport renewal for minor applicants 19 , accomplished by either parent or legal guardian
  • First-time passport applicant (minor) 19, accomplished by either parent or legal guardian
  • First-time passport applicant (adult – for lost passports) 19

 4. Original current passport AND photocopy of its data page

  • The passport data page is where the photo and personal details of the passport holder appears.

 5. Passport renewal fee: OMR 24.000 (cash only)

  • If a passport is expired for more than one (1) year upon its renewal, the applicant must accomplish an affidavit of explanation, with additional OMR 10.000 fee (cash only).

Additional Requirements:

  • For a minor applicant (17 years old and below):
- Copy of birth certificate (BC) issued by the Philippine Statistics Authority (PSA), formerly National Statistics Office (NSO)
- Copy of parents' passport data pages
- Accompaniment by at least one parent or legal guardian
  • For an infant applicant:
- Accomplished Report of Birth (ROB) forms 19 (in 4 copies)
- Photocopies of parents' passport data pages and labor cards (4 copies)
- DFA-apostilled PSA-issued Marriage Contract of parents (original and 3 photocopies)
- Copies of Birth Certificate issued by the Royal Oman Police (ROP) with English version, apostilled by the Ministry of Foreign Affairs (MOFA) (original apostilled                document and 3 photocopies)
- Additional fee of R.O. 10.000 (cash only)
- In case of late registration of birth (more than 30 days from the date of birth), the parent or legal guardian must accomplish an affidavit of late registration of birth, with    additional OMR 10.000 fee (cash only).
  • For a married woman who wants to use the surname of her spouse:
- Copy of Marriage Certificate (MC) issued by the PSA, apostilled by the Department of Foreign Affairs (DFA), if married in the Philippines
- If recently married abroad, in lieu of PSA-issued MC, copy of Report of Marriage (ROM) issued by the Philippine Embassy or Consulate where the marriage was            registered
  • For a married woman who wants to revert to the use of her maiden surname: 
- If widowed: Copy of the PSA-issued Death Certificate of the deceased husband, apostilled by the DFA
- If marriage is annulled in the Philippines: PSA-issued Marriage Certificate with annotation on the dissolution of marriage, apostilled by the DFA, or ROM with                    annotation on the dissolution of marriage. For annotation of the ROM at the Embassy where it was originally registered, the DFA-apostilled court order on the                  dissolution of marriage must be submitted, with additional fee of OMR 10.000 (cash only) for the annotation service
- If divorced or if the marriage was nullified in Oman: MOFA-apostilled copy of divorce or marriage nullification decree with English translation.
- If divorced or if the marriage was nullified in another country: Copy of the divorce or marriage nullification decree, with English translation (if applicable), apostilled by      the Foreign Affairs office of the country where the divorce or nullification of marriage took place.
  • For the replacement of a lost passport:
- Photocopy of the data page of the lost passport; if no photocopy is available, DFA-apostilled PSA-issued birth certificate of the applicant
- Photocopy of applicant’s resident’s card (front and reverse)
- Affidavit of loss explaining the circumstances surrounding the loss of the passport, with additional OMR 10.000 fee (cash only)
- Police Report with English translation
- Newspaper publication on the lost passport
- Additional penalty fee of OMR 36.000 (cash only)
  • For the manual extension of the validity of the passport:

Temporarily reinstated due to the COVID-19 pandemic, this service may be suspended again when the world situation returns to normalcy. For travel purposes, the applicant must check the regulations of the country he or she is travelling to, to ensure that the immigration authorities will accept a passport with manually extended validity.

  1. Only applicants who have already applied for passport renewal may also apply for the manual extension of their current passport’s  validity.
  2. Only passports with a validity of less than six (6) months can be extended.
  3. The manual extension of validity is only up to one year from the date of application.
  4. Below are acceptable reasons for applying for this service:                                                                                                                                                                      - medical emergencies (with proof of urgency such as a copy of medical certificate, confirmed flight details) requiring the OFW and                                                              - members of his/her dependent family to urgently travel to the Philippines or another country for medical treatment;                                                                                    - or urgent need of the OFW to renew his/her employment visa (with photocopies of front and reverse sides of applicant’s resident’s card or latest employment visa              stamp). 
  5. The Embassy will temporarily allow the manual extension of validity to an expired passport, provided that the applicant has already applied for passport renewal. The validity of the passport cannot be manually extended beyond one (1) year from the original last date of validity of the passportdeath in the family (with proof of urgency such as a copy of death certificate, confirmed flight details) requiring the OFW and members of his/her dependent family to urgently travel to the Philippines;

REMINDERS

  • The applicant will receive a text message from the Philippine Embassy when the new passport arrives from the Philippines. This text message will usually be sent out 4-6 weeks after the date of the applicant’s passport application.
  • Passports shall only be released to the applicant or his/her representative (with proper identification and written authorization signed by applicant). Exemptions are applicants below 18 years old whose passport can be claimed by the parent/guardian who submitted and signed the application. To claim the passport, bring the previous passport and official receipt, and authorization (for representative claimants). No passport shall be released without the said documents.
  • There is no need to book an appointment for collecting the new passport.
